Who We Are

Home care and home health are distinct types of care that are both provided in a home setting, but most people aren’t aware of the differences and use these terms interchangeably. The key difference is that “home care” is non-clinical care and “home health” is clinical care. There are also Agencies that provide home care and skilled services.

Care Connections At Home is a private duty skilled home health agency. Skilled private duty home care agencies provide skilled services by nurses, physical, occupational, speech therapists, and caregivers, usually called home care aides or certified nurse assistants, who are trained to understand the nuances of senior care. Home care aides can help older adults with activities of daily living, or just offer some friendly companionship.

Unlike Care Connections At Home and our sister company Coastal Breeze Home Health, “home care” is classified as personal care or companion care and is considered “unskilled” or “non-clinical.”

When To Use Private Duty Skilled Home Health

Situations in which home health is prescribed include:

  • Following an inpatient hospitalization or rehabilitation stay, to help a community member under the age of 65 transitions back to independence using expert clinicians. (Skilled Nursing, PT, PT SLP, MSW)
  • When a loved one has had a medication change, monitor for side effects and assure a new medication is effective
  • When a person experiences an overall decline in functioning – to help him/her regain independence through therapy or learn new skills to compensate for their deficit
  • When a person needs assistance with activities of daily living ranging from grooming to meal preparation
  • Meal preparation
  • Light Housekeeping
  • Help with dressing, bathing, and grooming
  • Transportation to appointments, running errands
  • Reminders to take medicine
  • Help with bill paying
  • When someone may be at risk for social isolation and needs companionship

Unskilled Home Care Agencies: Services provided by home care aides or companions only to assist with:

  • Meal preparation
  • Light housekeeping
  • Help with dressing, bathing, and grooming
